Complete These Steps Within Two Business Days of Your Employment Offer
Complete GSFC Form 24-27 Locator and Information Services Tracking System (LISTS) Data. We require this completed form and a copy of your Driver’s License to request a badge on your behalf. Download form here.
Child Sex Abuse History Check: Child Sex Abuse Background Investigation. Maryland law required that you complete one of these forms for EVERY job where you worked with children. Download form here.

Complete These Steps Within Three Business Days of Your Employment Offer
Finger Printing: We use the Live Scan fingerprinting process to perform a criminal history checks for all employees. Visit http://www.marylandfingerprinting.com/ to learn more about the process and find a location to have your fingerprints done. Our center code is 9000027131. MSDE’s code 1100000042. You must give the fingerprinting agency both numbers so that your criminal history results are sent to both us and MSDE.
Employment Verification Forms. Have your most recent employers (only those in the field of early childhood education) complete this form. You will need three verification forms if your experience indicates that you have had three jobs in the field of early childhood education . Download the form here .
Health Appointment: Contact your physician and have a medical form completed. You can download the form here. NASA requires an annual TB/PPD test. Please ensure that you have a current result as you cannot start work without it.
